How to Qualify for Help

Already working with a local organization?

You may already be eligible if you’ve been referred by:

  • CharityVision

  • Friends for Sight

  • Utah Partners for Health

  • Special Olympics: Opening Eyes Program

If referred by these groups, you likely don’t need additional paperwork—just bring your referral or voucher with you to your appointment.

Not connected to a partner organization?

You can still apply directly. We’ll help determine if you qualify based on:

  • Household income

  • Insurance status

  • Verification through a school, clinic, or food bank

Just give us a call or fill out our referral form to get started.
